Please choose the appropriate assessment from the menu on the right.  Please type your AUTHORIZATION CODE into the Password and User ID fields. If you have been instructed to take more than one assessment, the menu will remain visible while you take each one.  Simply click on the next assessment once you have submitted your answers and have seen the "confirmation" screen.  After reviewing the confirmation screen, use the "Back" button on your browser to return to the questionnaire and complete any unanswered questions or required information. Please use the TAB button, not the Enter key, to move between fields. Pressing the enter key will submit the form.
